By accessing this website, booking appointments, or receiving services from Évry Aesthetics Elmhurst (“Évry Aesthetics,” “we,” “our,” or “us”), you agree to be bound by these Terms & Conditions. If you do not agree, please discontinue use of our website and services.

2. Services Provided

Évry Aesthetics Elmhurst provides elective medical aesthetic and wellness treatments including but not limited to injectables, skin treatments, regenerative services, and cosmetic procedures. All services are subject to provider availability, medical appropriateness, and professional judgment. Individual results vary and are not guaranteed.

3. Eligibility

Clients must be at least 18 years of age to receive services. By booking, you represent that you meet these eligibility requirements.

4. Appointments, Cancellations & No-Show Policy

Booking

Appointments may be scheduled online, by phone, or in person.

Cancellation & Rescheduling Policy

We require a minimum of 24 hours’ notice to cancel or reschedule an appointment.

Late Arrival Policy

Clients arriving more than 15 minutes late may have their appointment shortened or rescheduled at provider discretion.

Deposits

Certain services, packages, events, or promotional appointments may require a deposit at the time of booking. Deposits are non-refundable. Deposits are applied toward the total treatment cost at time of service. Once product is ordered, deposits are non-refundable under any circumstances.

5. Payments & Refund Policy

Payment may be required at the time of booking, prior to service, or at the time services are rendered, depending on the treatment type, promotion, or appointment category.

We accept major credit cards, debit cards, and approved electronic payment methods.

Prepayments & Deposits

Certain services, packages, promotional appointments, and regenerative treatments may require full or partial prepayment to secure your appointment. Prepayments and deposits are applied toward your treatment balance on the day of service.

Refund Policy

All treatment services, prepaid services, packages, promotional pricing, and deposits are final sale and non-refundable.

Gift Cards & Promotional Credits

Gift cards and promotional credits:

  • Are non-refundable

  • Cannot be exchanged for cash

  • Are non-transferable

  • Must be redeemed before expiration (if applicable)

  • Cannot be combined with other offers unless otherwise statedRefund Policy

All treatment services, packages, promotional pricing, prepaid services, and deposits are final sale and non-refundable.

6. Membership Programs (If Applicable)

Membership benefits, pricing, discounts, rollover policies, and cancellation terms are subject to the specific membership agreement provided at enrollment.

Membership fees are non-refundable. Unused benefits do not carry cash value.

Évry Aesthetics reserves the right to modify membership terms with notice.

7. Medical Disclaimer

All services offered are elective aesthetic procedures and are not a substitute for medical care from your primary care provider.

Clients are responsible for providing accurate medical history, medication usage, allergies, and health information.

You agree to follow all pre-treatment and post-treatment instructions provided by your provider.

Results vary by individual and are not guaranteed.

8. Assumption of Risk & Limitation of Liability

By receiving services at Évry Aesthetics Elmhurst, you acknowledge and accept that aesthetic treatments involve inherent risks including but not limited to:

  • Bruising

  • Swelling

  • Redness

  • Pain or discomfort

  • Infection

  • Pigment changes

  • Allergic reactions

  • Unsatisfactory cosmetic outcomes

To the fullest extent permitted by law, Évry Aesthetics Elmhurst, its owners, providers, employees, contractors, and affiliates shall not be liable for indirect, incidental, special, or consequential damages arising from services rendered or website use.

9. Photo & Media Release Consent

By receiving services at Évry Aesthetics Elmhurst, you grant permission for before-and-after photos, videos, and treatment images to be taken for clinical documentation and marketing purposes unless you opt out in writing.

Marketing usage may include:

  • Website

  • Social media

  • Advertisements

  • Educational materials

Your identity will not be disclosed without consent. You may revoke marketing consent at any time in writing.

10. Communication Consent (SMS, Email & Phone)

Clients agree to contact Évry Aesthetics Elmhurst directly to resolve billing concerns prior to initiating a credit card chargeback.

Fraudulent chargebacks may result in account suspension and collection actions.

12. Promotional Offers & Model Programs

Promotional pricing, model call appointments, and special offers:

  • Are subject to availability

  • Are limited-time only

  • Cannot be combined with other discounts unless stated

  • Are non-refundable

  • May require photos or documentation participation

13. Intellectual Property
